Simultaneous robotic right middle lobectomy and thymectomy via a single-docking, supine approach: a strategic 5-port solution

The management of synchronous mediastinal and pulmonary pathologies presents a significant technical challenge, often requiring staged operations, intraoperative patient repositioning, or system re-docking. We report a case of a 67-year-old man with active myasthenia gravis and a synchronous right middle lobe pulmonary nodule who successfully underwent simultaneous robotic total thymectomy and right middle lobectomy during a single session. This single-docking robotic strategy maximizes the robotic system’s ergonomic advantages, eliminates the risks of repeated anesthesia in neurologically vulnerable patients, and offers an efficient alternative for multi-compartment thoracic resections.
